To make sure that Complete Freedom Osteopathy protects patients’ private information:

Complete Freedom Osteopathy respects the privacy of information obtained from our patients and we adhere to Australia’s privacy legislation when dealing with patients’ information (The Privacy Act 1988). The information collected includes contact details.

It is a requirement as a registered health professional in Australia for osteopaths to take treatment notes and to store these securely for at least 7 years. For children, they must be stored until they are 25 years old. Complete Freedom Osteopathy adheres to these requirements.
This personal information is obtained in many ways, including by paper forms, telephone and facsimile, by email, via our website https://www.completefreedomosteo.com.au/, from media and publications, from other publicly available sources, from cookies and from third parties.

All your information, including treatment notes, will be stored on secure password-protected servers. Paper forms are digitised and stored on these secure servers. The paper form is then shredded and recycled. Only your osteopath has access to your treatment notes. This information is strictly confidential and will not be shared with anyone, except for the following situations:
•    Your osteopath needs to update your GP or another medical specialist about your condition.
•    A court of law subpoenas your treatment notes.
•    Your osteopath believes that you or someone else is at risk of harm.
•    Your information needs to be shared as part of mandatory reporting requirements.

Any other sharing of your information would only occur with your written consent.

Where activities (such as administrative tasks) are outsourced by Complete Freedom Osteopathy, these companies are contractually bound to the same privacy levels we commit to our patients. Patients may request access to their information on Complete Freedom Osteopathy’s database. If any of this information is incorrect, the patient may request to have it corrected.
